Please follow these steps:
- Download our renewal form (attached at bottom of page).
- Fill out renewal form including payment details (eg. credit card info)
- Send to ARM Head Office the form, any required documentation and payment items (eg. cheque, if selected)
It should also be used if you just wish to update your existing details.
